I am willing to exempt the coordinating coaches, staff, and volunteers (all collectively referred to as Wild Adventure World Limited) in this course.

I understand that the course involves physical activity and is strictly required. As far as I know and believe, there is no health or other reasons that would prevent me from participating in this training and being assessed. I am in good health and do not have any existing health or mental problems or allergies.

I have basic swimming ability (meaning that I can swim 30 meters on my own without help from others), and no medical practitioner has ever made any objection. `

I voluntarily participated in this course, and understand that some training items are dangerous. If you get injured or encounter an accident while participating, the person who arranged the course will not be held responsible.

I know and understand that participating in any course of Wild Adventure World Limited will involve many risks and dangers, and the degree may be greater or different in nature than what I encounter in my daily life.

I am willing to accept and fully bear the risks, crises and dangers and risks, personal injuries, deaths, property damages or losses that may be caused by crises and dangers.

Give up any and all claims, legal actions, recovery of expenses, recovery of expenses and demands that I may make against Wild Adventure World Limited.

Wild Adventure World Limited is exempt from responsibility for any loss, damage, injury or expenses incurred by me or my family members for any reason due to my participation in the itinerary/course.

I agree and allow Wild Adventure World Limited to take photos or videos during the itinerary/course for recording and promotional purposes.

I agree and allow Wild Adventure World Limited to use my personal email for future promotion: notices of classes, interest classes, workshop gatherings, exchange plans, tours, hiking meetings, seminars and other activities.

I agree to follow the guidelines and instructions of Wild Adventure World Limited before and during the itinerary/course.

I agree that this health statement is regulated by the laws of Hong Kong, and any related legal issues will be handled by a fair court with jurisdiction in Hong Kong.
